- Please provide your real name or a decent nickname. If you use a name containing special characters such as underscores, hyphens, or numbers etc,
I reserve the right to edit it as I see fit.
*Note: The use of last names is NOT required or recommended. - If you have a website you must put a code on your site or I will not list your site on the members page. That being said, having a website is certainly not a requirement.
- You must have a valid email address or the form will not go through.

Feel free to create and use your own codes. If you'd like to submit your codes for others to use here, email them to afanatic(at)gmail(dot)com or use this form to contact me. Codes must be standard button dimensions (50x50, 88x31, 75x50, 100x35, 100x50). Credit will be given, and codes will be uploaded to our own server.
